If the fist document contains some grafik named "image1" and the second document contains also some grafik with the same name will be appear the error message "Error while reading the file"

Test case:
- create textdocument with some grafik named "image1"
- create a second textdocument with some grafik with the same name
- click to the menu item "insert file"
- error message

Tested too with Libreoffice 4.1.4 RC1

thanks for the issue. I can confirm this.
First occurence is in 4.0.6.1 (as far as I can check).

Production of the "Read error" message was introduced in the period covered by the 40+ bibisect repository.  But surely the real problem is the failure to complete the requested insertion.  that failure goes back at least to version "oldest" of the 40 bibisect repository.

same problem with Insert File as with opening a file that contains same frame name twice.
