Problem description: 

Steps to reproduce:
1. create a new document, Format->Page->Page tab, select "DL Envelope" as format, and Landscape orientation.
2. start the mailmerge assistant, use current document, Letter,  select any address list, do all the necessary things (field matching etc) until step 6 "Edit document" - do this, you see that the correct page format is done, it is a DL Envelope.
3. Now go to "Next>", to step 7; it renders the pages, and you can look at all at once.

Current behavior:
The first page now is an A4 page here (my country default), and from the second to the last are DL Envelopes.


Expected behavior: ALL pages should be DL envelopes.

This malfunction renders the complete mailmerge assistant useless.

Confirmed on V: 4.2.2.1 release, OS: Win7 64

Also applies to general styles & formatting, 1st page defaults to default.

Work around (for styles & format at least) create a blank mailmerge entry at position 1 that can be dropped from the mailmerge post compilation

AFAICT it IS a dupe of bug 69282.
Nevertheless, I now use debian sid with LO 4.3.3.2 - in this version I can not reproduce the bug any more. So for me (as reporter) you can close this bug.
