Make an icon and put save it somewhere.
Start Draw or another module of LO.
Open 'Customize Toolbar' dialog and add a command, which has no icon yet, e.g. 'Extended Tips' from category 'Application'.
Click on 'Modify' and then on 'Change Icon'.
Click on 'Import' and select your icon.
You get the attached error message, although the icon is actually imported.
Assign the imported icon to the command.

which file type did you use for your icon? The dialog's Help says

You can only import icons that are in the PNG file format and that are 16x16 or 26x26 pixels in size.

(but this does not change anything. If I use a PGN file with a size of 16x16, I get the message

file /path/to/file

in the box below, but with no further info ... :( ).

I have used file type .png. I have tried both sizes 16x16 and 26x26. I'm sure the file format has been correct, because the icons are actually added. Only the fact, that an error message pops up, is wrong.

[To add an 16x16 icon, you have to set the icon size to 'small' in Tools > Options > LibreOffice > View. To add an 26x26 icon, set the icon size to 'large'.]

Confirmed in 4.1.6, 4.2.6 and master on Linux. This doesnt happen with 4.0.6.

@Noel: It's a wrong conversion of the css::ui::ImageManager service in 0394cb37a2d378511f265004b285fd93df37e5ca. It must be initialized with a Storage.

Noel Grandin committed a patch related to this issue.
It has been pushed to "master":


fdo#76658 fix bad conversion of old-style UNO service..

It will be available in 4.4.0.

The patch should be included in the daily builds available at
http://dev-builds.libreoffice.org/daily/ in the next 24-48 hours. More
