Example document showing the bug

Problem description: 
When creating a TOC without changing any of the settings, there's no spaces between the chapter number and the chapter headline..

Steps to reproduce:
1. Create new Writer document
2. Create a level 1 Heading and a level 2 one
3. Enable outline numbering
4. Insert TOC with default settings

Current behavior:
There is no whitespace between outline numbering and headline in the TOC, which makes it hard to read.

Expected behavior:
There should be a space.

The Bug Submission Assistant wouldn't let me select 4.3.0.4.
