An ODT file that demonstrates Headings that not appearing for cross-reference selection

Problem description: 
Headings are not generating Cross References. I have attached a reference .odt file that exhibits the problem.
In highlighting the problem, I copied content from another document that had _some_ missing heading cross-references. I deleted that copied content within the attached document, but the problem has been inherited in the new document.
I have been unable to isolate the cause nor correct the behaviour in the original documents, nor attached document.

Steps to reproduce:
1. Open attached 'CrossReferencesMissingForHeadings.odt'
2. Press F5 (View > Navigator)
3. None of the included Headings are listed for cross-reference

Current behavior:
None of the assigned Headings in the attached document are available to assign as cross-references. 

Expected behavior:
Headings to appear in Navigator, and be assignable as cross-references via 'LOWriter > Insert > Cross-references > Headings'

In Styles and Formatting, Heading 1, 2, 3, and 4 have been removed from Outline Numbering and they do not inherit from Heading (doesn't affect outlining).  Go to Tools → Outline Numbering and add each heading to the corresponding level.

Changed to RESOLVED WORKSFORME.
