When creating radio buttons in a Writer form, all radio buttons get thrown into one bunch (despite them being separated in the Writer document).

As you will see, 2 buttons are selected by default, but as soon as you start clicking on the radio buttons, you'll only be able to select 1

the bug is fixed (and it's a duplicate, didn't find it before)
