1.) new document
2.) write a long text to the first paragraph
3.) add a empty paragraf
4.) write a long text to another paragraph
5.) add an image with caption to the empty paragraph

6.) mark & copy starting from the middle of the first paragraph to the middle of the last paragraph (image included)
7.) paste it sometimes at the end of the document

8.) mark & copy starting from the middle of the third-last paragraph (=text) to the middle of the last paragraph (=text) (last image included)
9.) paste it sometimes at the end of the document

10.) Check the image numbers

Could not reproduce, the numbers run correctly and do not get stuck like in your document.

Btw., in your document you have image 2 on top of image 1 for some reason.

By "sometimes" do you mean "several times"?

yes: sometimes = several times

To reproduce you have to follow the steps exactly. If you do so, image #2 will be rendered on top of image #1!

Can you please try this finer steps to reproduce?

1.) new document
2.) add text to the first paragraph ("aaa aaa aa aaaa aaaaa aaa" ...)
3.) add a empty paragraf 
4.) add another paragraph with text ("aaa aaa aa aaaa aaaaa aaa" ...)
5.) add an image to the empty paragraph, add caption (adds image-number-field)

6.) mark & copy starting from the middle of the first paragraph to the middle of the last paragraph (image included)
7.) Set Cursor to the end of the document after the last character
8.) paste clipboard several times at the end of the document

9.) mark & copy starting from the middle of the third-last paragraph (=text) to the middle of the last paragraph (=text) (last image included)

10.) paste it several times at the end of the document
12.) Check the field numbers

Thanks, now I got the numbers to get stuck when doing the last paste series (step 10).
Yet, I didn't get #2 on top of #1.

2. Test your bug
3. Leave a comment with your results.
4a. If the bug was present with 3.3 - set version to 'inherited from OOo';
4b. If the bug was not present in 3.3 - add 'regression' to keyword
