If you save a presentation in outline view and then open it up, the slide icon that appears next to each slide number is missing

Steps:
1) Open attachment 120895 [details]
2) Notice that there isnt a slide icon next to the slide title '1, 2, 3...'
3) Switch to normal view and back to outline view and the slide icon will be there

Still repro with the attached file, but now I can't seem to make new files open in outline view.
