Yesterday, I was updating a document with this new version  and trying to
update my table of content, it completely messed it. I found no way to make
the page number be numbered as 1,2,3 instead of A,B,C.

In addition it prefix each table index entries by the entry number.
Which also looks terrible...

Thanks for filing, Stéphane.
But for those issues really a test file is needed. And probably it's a special case, document ..
