Created attachment 134945 [details] Example of Radial Forms When grouping radial forms for the purpose of a survey, the grouping breaks when it is exported to .pdf Steps to Reproduce: 1) Create radial forms. 2) Give each form group a group number so that the circles can be filled in for each question without undoing the previous answer. To do this, make sure that design mode is "on" via the Form Controls toolbar. Select a group of radial controls and right-click them and select Control. Go to the general tab of the window that pops up and select type a unique name/number for that group. Go to the other groups and use a different unique group name/number for each group. 3) Export to .pdf 4) Open .pdf in Adobe Reader, Okular, or any other PDF Reader. 5) Left click a radial form for each group and watch it undo the previous one. Inspection using Adobe Reader Pro showed that it couldn't see any group name/number as if that data it wasn't exported. This has been reproduced in both Windows and Linux and in 5.4.0.3
I'm not sure if it's relevant, but when opening the saved forms (not pdf) in Microsoft Word 2016, it failed to even render the forms.
Not reproducible for me with LO 5.4.2.0.0+ built at home under Ubuntu 16.04 x86-64. Please could you attach your own pdf and a screencopy of the option dialog of your pdf export? Set status to NEEDINFO, please set it back to UNCONFIRMED once requested informations are provided. Best regards. JBF

In the ODF document each line is a group and in the pdf there is only one group. Setting as NEW. Did you try to play with the group names ? For example using names starting with an alphabetical character instead of numbers? Best regards. JBF
I just tried using names (one, two, three) instead of numbers (100, 200, 300) and it didn't work. I've also tried exporting to .pdf from different file types like .doc and .docx. That didn't work either.
