I had the problem reported in this openoffice.org bug (http://openoffice.org/bugzilla/show_bug.cgi?id=109741). To reproduce: after you insert a header in a text document and save it in DOC format, close
and open the file again, a footer is automatically created.

This bug was filed before the changes to Bugzilla on 2011-10-16. Thus it
started right out as NEW without ever being explicitly confirmed. The bug is
changed to state NEEDINFO for this reason. To move this bug from NEEDINFO back
to NEW please check if the bug still persists with the 3.5.0 beta1 or beta2 prereleases.

[REPRODUCIBLE] with LibreOffice 3.5.3.2 (Build-ID: 235ab8a-3802056-4a8fed3-2d66ea8-e241b80), German UI, on MacOS X 10.6.8 German.

Following these steps similar to the original description:
1) Start LO.
2) Create a new Writer document.
3) Add a header (but no footer!).
4) Save the document as .doc (Word 97/2000/XP/2003) file.
5) Close the document.
6) Open the document again:
7) Header AND footer are present.

Not terrible, but annoying, because it may break the page layout.

Could not reproduce. Seems footer is not added.

I too cannot reproduce (using LibreOffice 4.2.4).
