Sample document which illustrates the incorrect behaviour

Problem description: 

Pages with even simple graphics (e.g. borders on paragraphs; table borders; simple diagrams) do not print properly. The first page printed does seem to work sometimes, but second and later usually pages don't. The graphics are usually missing, but can also be at the wrong location and/or incorrectly sized. 

Steps to reproduce:
1. Print the attached document.

Current behavior: (described above)

Expected behavior: printing of what I see on the screen.

              
Operating System: Windows 8
Version: 4.0.1.2 release
