This may not be a LibreOffice problem but worth being aware of:
Printer is a Lexmark c543DN on local network.  Libreoffice Writer created document on MacBook Air which printed perfectly, taking paper from tray.  However printing the envelope failed; just one line of the address printed across the top edge of the DL envelope, fed from manual feed slot.  Transferred the document to a Windows PC, connected to the same printer, and the envelope printed perfectly.

Can you please provide a test document so this can be tested against and subsequently be confirmed.

Also have you update LO meanwhile? Does 4.1.3.2 still have this odd behavior? Which OS X are you on?
