When export to docx, hyperlink and alternate text at the image are lacked.

I can confirm those problems after filesave & reopen:

- Missing 'Alternative (Text only)'
See: right-click at 2nd picture > Picture... > tab Options > Alternative (Text only)
Perhaps same as 'Title' on Description > Title (Bug 43546)

- 'Name' replaced with 'Picture' & 'graphics1'
See: right-click at pictures > Picture... > tab Options > Name

- Missing hyperlink
See: right-click at 1st picture > Picture... > tab Hyperlink > URL

Tested on LO 4.0.4.2 (Win7 32bit)

Indeed, is a duplicate bug
