In the new version, whenever I change text to italic, it changes it extended characters.

This may or may not be related to: 96116

I tried reinstalling, as well as reinstalling all fonts.

To me, this is a duplicate of bug 96116. What your screenshot shows are not actual diacritics (is that what you mean by “extended characters”?), just pieces of other characters.

OpenGL is going to be disabled by default again in 5.0.4, by the way.

Yep, definitely seems like a duplicate.
Disable Tools - Options - LibO - View - Use OpenGL for all rendering and restart LibO.
